How to Apply
All long-term care homes in Ontario including the Perley and Rideau Veterans' Health Centre process admissions through their local Community Care Access Centres. The Champlain Community Care Access Centre coordinates the placement of clients into long-term care homes and provides in-home health and support services in the Ottawa area.

Once you have decided to go forward with an appli ca tion to long-term, respite or convalescent ca re, please contact the Champlain Community Care Access Centre at 613-745-5525. A Case Manager assigned to you will guide you in gathering the medi ca l and personal information required for admission. She may offer help in choosing a home, if you require it. The Case Manager ca n provide an estimate on wait times.
